NYC Health + Hospitals | Gotham Health Renaissance Health Care Network Diagnostic and Treatment Center is the hub of nine conveniently-located health centers in Central and West Harlem and Northern Manhattan. Together they provide the gamut of primary care services, including free pregnancy testing, prenatal care and delivery, well-child care, cancer screening, treatment of chronic illnesses such as asthma, diabetes and hypertension, dental services and immunizations.

• In collaboration with the medicine practice manager, nursing supervisor, chief of medicine and the patient centered medical home (PCMH) care teams; design, initiates and facilitates PCMH workflow processes and monitors PCMH compliance. Additionally, oversees the collection, integration and evaluation of diverse data related to clinical and operational performance and medical home transformation activities
• Responsible for being a subject matter expert on NCQA requirements and updating team on any updates or changes to PCMH standards and recommends best practices
• Oversee the medical records department ensuring compliance with standards of accrediting and regulatory agencies
• Assist with developing policies and procedures for documenting, storing and retrieving information and for processing medical-legal documents, insurance data, and correspondence requests in adherence with federal, state and local statutes
•  Oversee the referrals department including identifying all workflow issues and streamlining processes to reduce barriers that improve the timeliness of patients appointments and patient satisfaction
• Maintain a consistent focus on patient experience that includes providing high quality customer service
•  Collaborates with senior leadership team to develop and monitor performance improvement initiatives and goals
• Coordinate staff schedules ensuring adequate coverage across assigned departments
• Monitor the environment of care by making periodic rounds and coordinate with appropriate departments to maintain cleanliness and safety. 
• Participate in staff engagement activities to create a positive culture and morale.
• Take part in departmental team huddles and meetings and provide administrative updates as needed.
• Actively participate in leadership huddles, meetings and provide support to colleagues including but not limited to providing administrative back-up coverage to other departments in the event of both scheduled and unscheduled leave.
• Assist with coordinating major projects as it relates to the department of information technology (I.T.)
• Rotate the administrator on duty (AOD) responsibilities ensuring that clinic operations run smoothly and resolving any issues that arise. AOD coverage includes at least one late clinic night and Saturday coverage.
• Complete annual employee evaluations and meet regularly throughout the appraisal year offering timely performance feedback to staff
• Execute any other duties assigned by the Associate Executive Director of Sydenham.

EDUCATIONAL LEVEL: Bachelor Degree

YEARS OF EXPERIENCE: 5 years
