NYC Health + Hospitals | Gotham Health Morrisania Diagnostic and Treatment Center serves the South Bronx through a two-pronged approach: providing treatment to those who need it, while at the same time emphasizing prevention. Morrisania runs one of the oldest School Health Programs in New York City and serves four schools through five school-based clinics. These clinics ensure access to free primary medical, dental and mental health care to children who have unmet health needs.

 



Job Description

PURPOSE OF POSITION

The Community Liaison Worker Level II serves as an outreach worker to identify hard to reach, vulnerable and underserved populations. He/she will develop and implement community outreach activities to ensure that individuals are promptly connected to primary care and other medical services at Gotham sites and off-sites, otherwise known as, Community Health Centers. The Community Liaison Worker also serves as a liaison between the community, local organizations and Gotham Health sites and facilities. The Community Liaison Worker will be responsible for connecting prospective patients with multiple disciplines across the continuum of health services offered by Gotham Health and its community partners. Increasing new business at various Gotham sites, increasing new business for Covid 19 sites

AREAS OF RESPONSIBILITIES

Under direction, performs difficult and responsible liaison activities between City agencies or programs, and community residents and groups. In the Gotham Health Corporation, under direction, supervises liaison activities of a hospital with local residents and other agencies and groups in the community. In addition to performing duties of Assignment Level IV at a higher level, also performs the following:

SUMMARY OF DUTIES AND RESPONSIBILITIES

  • Recruits new patients for all Gotham Health Centers and sites by attending community events, local meetings and developing partnerships.
  • Distribute Gotham Marketing and Flyers.
  • Working with local business to increase membership and raise awareness about Gotham Health
  • Track number of new patients
  • Collaborates with community-based partners to ensure coordination of clinical and community-based organizations.
  • Conduct patient and client recruitment, patient navigation
  • Performs field or office tasks as assigned.
  • Maintains flexibility to work nights and weekends to accommodate outreach events.
  • Other duties as deemed necessary.


Minimum Qualifications

Education

Associates Degree

High School Diploma

 

For appointment Level I

1. One year of full-time experience in a government or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or

2. Completion of an approved six month training program combined classroom and on-the -job experience in community liaison work, plus six months of full-time experience as described in 1 above.

For appointments to Level II

In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires:

1. Two additional years of full-time experience in counseling, community work, or community health activities in government agencies or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations, or other government agencies for the purpose of providing assistance and obtaining participation and support for implementations of community of public service programs; or

2. Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of a 30 semester credit for each the experience described above. However, all persons must have at least one year of the full-time experience described above.

For appointment Level III

In addition to meeting the qualification requirements of Assignment Level II, appointment to Level III requires:

1. One additional year of full-time experience as described in 1 above

For appointment to Level IV

In addition to meeting the qualification requirements for Assignment Level III, appointment to Level IV requires:

1. One additional year of full-time experience.
