Job Description

Tour I: Rotating
Purpose of position: Under direction, with moderate latitude for the exercise of independent initiative and judgment , provides professional pharmacy services in a hospital, clinic or other health care setting.
Areas of Responsibilities: All areas in Kings County Pharmacy Department
•Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows up on problems. •Enforces policies that address medication use throughout the institution.
•Compounds sterile and non-sterile medication according to USP 795 and 797 guidelines.
•Orders, receives and distributes controlled substances. This includes identifying and resolving discrepancies. •Operates and troubleshoots automated dispensing systems, as needed.
•Escalates serious or emergent matters that require action by a supervisor or director.
•Notifies pharmacy informatics and/or contacts the appropriate vendor or helpdesk to address information technology failures.
•Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
•Provides and documents drug information and patient counseling.
•Documents interventions, adverse drug reactions (ADRs), and medication errors.
•Directs and utilizes personnel including technicians, aides and clerical staff.
•Participates in Pharmacy staff in-services and hospital wide programs meetings.
•Operates and troubleshoots automated dispensing systems, as needed.
•Advises prescribers on alternatives for non-formulary drugs. In the absence of the director’s timely approval, the pharmacist may approve the use of nonformulary medication provided all other aspects of the policy.
•Utilizes Outpatient and Inpatient computerized systems to enter and access patient care information.
•Coordinates and analyzes statistics and/or clinical data.
•Participates in quality management and utilization review programs.
•May coordinate the work of a small group to accomplish a project or assignment.
•May participate in the preparation and review of policies and procedures.
•May assist in the development, implementation and execution of Pharmacy staff training curriculum.
•In the absence of the supervisor, may assist in the supervision of a Pharmacy unit or function. This includes arranging of unscheduled coverage for a subsequent tour.
•Complies with KCHC and departmental polices for safety, quality improvement and infection control.
•Provide patient/family education appropriate to age.
•Adheres to organizational policy for annual in-service education and health assessment.
•Maintains confidentiality of patient and hospital records



Minimum Qualifications

Possession of a valid NY State license to practice pharmacy.



Department Preferences

Educational Level: R.Ph. Bachelor Science or PharmD

Equipment/Machines Operated: Parata Max, Pyxis, and CII Safe

Computer Programs/Software Operated: Epic, Opus, Simplifi and Pyxis
