The Bereavement Coordinator's responsibilities includes:

Provides and documents individualized clinical assessment, care planning, intervention and outcome
evaluation for bereaved families and/or caregivers of Hospice patients.  Provides general bereavement
support via community outreach and education. As a member of the interdisciplinary team, promotes
effective grieving by providing access to appropriate bereavement care and services for
families/caregivers of hospice patients.  Also provides bereavement support through community outreach
and education, including support groups, special events and phone support.


Experience in bereavement work is required; Master’s degree in Social Work, Counseling, Psychology, pastoral care, marriage and family counseling, or other related field is required; State licensure in Social Work, Psychology, Counseling, Marriage and Family Therapy, or other related discipline, or course work or continuing education specific to loss/grief/bereavement is required.

The territory that this position will cover is Upper Bucks County and Northeast Philadelphia.
