NYC Health + Hospitals/Gouverneur provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly and infirmed in its 295-bed Skilled Nursing Facility (SNF). Gouverneur recently completed a $250 million modernization project to offer enhanced, cutting edge healthcare services in a state of the art space.  NYC Health + Hospitals/Gotham, Gouverneur’s D&TC is the largest freestanding D&TC in New York, providing more than 250,000 visits per year on the Lower East Side. Gotham, Gouverneur provides a comprehensive array of on-site services, including primary care, pediatrics, women’s health, dentistry, behavioral health, podiatry, optometry, ENT, digital imaging, infectious disease, and other specialties and sub-specialties.   NYC Health + Hospitals/Gouverneur Skilled Nursing Facility provides 24-hour care for individuals in need of short-term, long-term care or hospice care.  The facility is rated as a Best Nursing Home by US News and World Report.  Post-Acute Care services include physical therapy, occupational therapy, speech and audiology, and vocational rehabilitation. 

Job Description

1. Assist with and provide physical care to residents according to established standard of performance.

2. Performs and records assigned procedure and nursing care activities completely and skillfully, such as, BP, TPR's, height, weights, changing simple dressing, feeding and other related nursing duties. 

3. Maintains the environmental conditions and practices to ensure comfort, safety, and security for residents property.

4. Demonstrates safety awareness and follows protocol for fire, disaster, infection control and other environment of care practices.

5. Maintains knowledge and skills through participation in conferences and planning for patient care. Attends all mandatory classes.

6. Participates in performance improvement activities. 

7. Demonstrates a sensitive and supportive attitude towards the resident, family, and/or significant others.

8. Assists in recreational and rehabilitation acitivities designed to meet the assessed needs of residents. Supervises residents during field trips. 

9. Report observations of paitents' and families' condition, mental state and social behavior to medical and professional staff caring for patients, and participates

in the assessment and/or care of patients and families. 



1. Graduation from elementary school.
2. Successful completion of a basic skill training program for Nurse's Aides, or its equivalent.
