Job Description

Marketing Statement

At Home is a division of New York City Health and Hospitals which provides community-based health care services including: Health Home and Care Coordination, Certified Home Health Care (CHHA)/ Care Management, and Outreach/Engagement. We have over 400 clinical and non-clinical staff embedded throughout the NYC H+H enterprise who service over 9,000 patients located in group/private homes, as well as those without housing in the four boroughs of New York City.


Job Description

This position will be based at the NYC Health and Hospitals/Kings County worksite


1.  Comforts patients, reassuring them about recovery and explaining the process of how acute distress is addressed.


2.  Provides orientation and support to patients and collaterals.


3.  Helps patients to understand their role in their own recovery process.


4.  Uses personal experience as a recipient of behavioral health services (including co- occurring/substance use background) in role-modeling recovery and providing hope for other recipients.


5.  Assists patients in seeking clarification about the treatment and recovery process.


6.  Engages patients and helps motivate them to enter treatment and engage with care coordination.


7.  Provides motivation to address ambivalence about change using basic motivational interviewing techniques.


8.  Conducts overdose prevention trainings with patients and their families, including naloxone distribution.


9.  Provides linkage to continuing care and community resources.


10. Conducts follow-up with patients after discharge.


11. Conducts outreach and connection to services for a period after discharge.


12. Assists with patient documentation, tracking, and follow up.


13. Works with OPD treatment team to facilitate engagement with clinic services.


14. Collaborates with staff in maintaining appropriate documentation.


15. Participates in supervision, department staff meetings, and other program review meetings.


16. Receives supervision and support from substance use disorder treatment program and administrative leadership. In addition, receives task supervision from team leads with whom they work on other services.


17. Other duties as assigned.



Minimum Qualifications

1.  A four-year high school diploma or its educational equivalent; and


2.  Satisfactory and documented completion of a formalized peer counselor/specialist training program which meets the training

requirements for a Peer Advocacy Organization as mandated by the State of New York Office of Mental Health in their Official Policy Manual. The training program must be at least six months in duration; or


3.  One year of full-time satisfactory experience working with consumers of mental health services as a peer counselor/specialist, peer advocate or in a related human services area.
