Job Description

Job Summary:

The Registered Nurse will provide professional nursing care including diagnosing and treating human responses to actual and potential health problems through case finding, health teaching, counseling and the provision of care supportive of or restorative to life and wellbeing in accordance with their scope of practice.

SUMMARY OF DUTIES AND RESPONSIBILITIES :

  • Demonstrates effective interpersonal, verbal and written communication skills.
  • Assume responsibility of Charge Nurse when necessary or assigned.
  • Complete written daily assignment for individual treatment team as dictated by general clinic schedules.
  • Completes a nursing assessment utilizing the initial and the reassessment as appropriate.
  • Review electronic medical record template for scheduled patients.
  • Maintains nursing panel and other daily processes.
  • Follow up abnormal labs and recall patient as per provider’s instructions.
  • Document all pertinent information on-line in the Nurses Contact Note, assessment/screening modules as appropriate. Document all patient contact, interaction and education in the electronic medical record.
  • Documents in the paper medical record when the electronic system is down. Utilizes appropriate downtime documentation procedure when the electronic system is down.
  • Screen patient waiting area and determine the acuity/priority needs of patients.
  • Supervise nursing licensed and unlicensed support staff.
  • Triage patients in person and via telephone for care, treatment or medication needs.
  • Collaborates with interdisciplinary team to provide appropriate health care needs based on age and medical condition of the patient. (cultural and language sensitivity)
  • Assumes responsibility for safe work practices and adheres to corporate patient safety policies.
  • Carries out accepted Infection control protocol (practices) as per Network policy. Facility specific Infection Control Plan and policies
  • Adheres to maintaining HIPPA standards as per Network policy.  Conform to policies governing patient rights and confidentiality of patient information (HIPPA)
  • Adheres to Network standards for Customer Service during daily activities with patients/family and staff.
  • Ensures that emergency equipment is available and functioning and checked daily, i.e. AED.
  •  Responds to all Rapid Response occurrences as assigned or if present in the area.
  • Participates in departmental and interdisciplinary conferences pertaining to policies and procedure affecting nursing practice.
  • Participates and utilizes ICARE model and PDSA in Quality Improvement projects (Nursing, departmental, service based).
  • Participates in the development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel.
  • Knowledgeable and upholds to the Gotham Health Standards of Behavior.

Pre-Visit Reviews – Chronic Disease Management :

  • Participates in Treat to Target, Collaborative Care by reviewing scheduling templates in collaboration with PCMH.
  • Review preventative health needs of the patient.
  • Identify any new issues/concerns from record review, telephone communication or team
  • Check chronic disease status and requirements for care
  • Check for immunization status, asthma care management.
  • Follow-up on patients who require further monitoring for care/treatment rendered. Facilitate care coordination across the continuum of care including follow up of post discharges and transfers
  • Provide self-management tools including, but not limited to home monitoring equipment (B/P machine, glucometer, etc).
  • Delegates and assigns those components of nursing process as appropriate which may be carried out by allied nursing personnel.
  • Leads and actively participates/conducts in daily huddles and team briefings.
  • Oversees the preparation of Pre-visit Planning completed by the PCAs.

Post Visit – Patient Adherence to Care Plan

  • Calls and contacts patient as needed.
  • Review/discuss self-care processes and goals with patient/family.
  • Assess learning needs and barriers that will affect meeting care plan goals.
  • Develop the care plan based on goals established by team/provider.
  • Identify barriers to care and assist with resources and referrals as appropriate.
  • Provides culturally sensitive care to patients and families in collaboration with other health team members.
  • Implements the nursing process utilizing assessment skills and strategies to plan for the care of the patient/family based on identified needs, problems or provider orders.
  • Reports all communicable diseases/conditions to the Department of Health.
  • Provide procedural, referral visit instructions including facilitating appointments to ophthalmology, podiatry, nurse visits, etc.
  • Conduct nurse visits as referred by Provider or as deemed necessary to monitor patient for care/treatment.
  • Provides teaching, guidance and direction to allied nursing personnel in rendering patient care.
  • Coordinate patient care with interdisciplinary team or community agencies as appropriate.
  • Ensure that LOOK ALIKE SOUND ALIKE and HIGH RISK medications are separated.
  • Administers and records medication and treatment administered to patients as ordered by the provider.
  • Evaluates administered medications for any immediate side effects before discharging patients.
  • Administer injections, immunizations, PPD and medications as ordered or by standing orders.
  • Provide pertinent hand-off communication to team members, Emergency Department, diagnostic services or to In-patient nursing staff when patients are being transferred or admitted to other services.
  • Obtain vital signs or reassess as necessary.
  • Performs or delegates POCT as needed.
  • Conduct health teaching, instructions and counseling sessions to patient/client and or significant other as needed utilizing motivational interviewing skills.
  • Perform Cardiopulmonary Resuscitation (BLS) when necessary.
  • Encourage patient/client to vent emotions in a socially acceptable way. Provide support to the patient in expressing concerns and fears and develop plan to address including referrals to other disciplines as indicated.
  • Procure medications and supplies for the Practice. Ensure practice is adequately stocked with necessary supplies.
  • Check and remove expired medication from cabinet and refrigerator.
  • Reports (incidents) occurrences immediately and completes (incident) occurrence form when necessary. 

Personnel Responsibilities

  • Orients licensed and unlicensed nursing staff to the Practice and Ambulatory Care.
  • Responsible for personal growth and development, developing leadership skills and maintaining clinical competence by attending continuing education programs.
  • Completes annual mandates in-services, OHS (employee physicals),
  • Maintains current licensure and BCLS/ACLS/PALS certifications.
  • Arrives in work area at designated time. Comply with organizations policy for time and leave.
  • Completion of the Ambulatory Care Nursing Core Curriculum.
  • Maintain log sheets and databases as needed/requested.
  • Performs assigned responsibilities accurately and in a timely manner.
  • Conforms to policies governing dress/uniform code, attendance and punctuality.
  • Performs related duties as assigned.


Minimum Qualifications

QUALIFICATIONS FOR THE JOB:

CERTIFICATION(S)/LICENSE(S):

  • NYS RN License
  • Certification in Ambulatory Nursing preferred

EDUCATIONAL LEVEL:

  • Accredited Nursing program
  • Associates in Nursing;
  • BSN degree from an accredited school preferred
  • Certification in Ambulatory Care preferred

KNOWLEDGEABLE IN:

  • Ambulatory and/or primary care nursing
  • Spanish or a second language preferred

YEARS OF EXPERIENCE:

  • 2 years or more in primary care
  • 1 year ambulatory nursing preferred
