NYC Health + Hospitals | Gotham Health Morrisania Diagnostic and Treatment Center serves the South Bronx through a two-pronged approach: providing treatment to those who need it, while at the same time emphasizing prevention. Morrisania runs one of the oldest School Health Programs in New York City and serves four schools through five school-based clinics. These clinics ensure access to free primary medical, dental and mental health care to children who have unmet health needs.

PURPOSE OF POSITION:
Under supervision of the Head Nurse, provides professional nursing care to patients within their assigned unit. Using established evidence-based guidelines diagnoses and treats responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health
counseling and the provision of care supportive to and restorative of life and well-being.  Coordinates care for assigned Care team, supervises the Care Team Ancillaries and collaborates with the Primary Care Provider.

SUMMARY OF DUTIES AND RESPONSIBILITIES:
•    Identifies, assesses, evaluates patient response to illness, through screening, observation, signs and symptoms and verbalized chief complaints
•    Documents all pertinent information in Electronic Medical Record.  Formulates and implements a plan of care, based on identified problems/needs with the patient/significant other
•    Sets priorities and provides support through direct care, health teaching/coaching, counseling, self-management techniques and referrals
•    Collaborates with other team professionals to evaluate patient health care needs and to ensure continuity of care
•    Responsible for Pre-Visit Planning, Collaborative Care, STI screening, Exit Interview for a group of selected patients, and Asthma action plan education
•     Assists with diagnostic tests/ procedures and the administration of medications/vaccines, standing order medications and treatments, and ensures that PCAs perform POC testing according to policy and guidelines.
•     Interprets and reports responses of patients/clients to appropriate members of the health team
•    Recognizes emergency situations and functions effectively, according to emergency protocol
•    Determines and assigns those components of the nursing regimen which may be carried out by the Licensed Practical Nurse and Patient Care Associates
•    Participates in defining, implementing, maintaining and interpreting standards of nursing practice
•    Participates in departmental and interdisciplinary conferences pertaining to policies and procedures   affecting nursing practice. “in-charge” responsibilities when assigned and/or necessary
•     Assists in the orientation of new staff
•    Participates in quality improvement data collection, outreach and health fair activities
•    Carries out and upholds Patient Safety and Infection Control Protocols
•     Fosters good Customer Service in daily activities, when dealing with general public and staff

For individuals holding an unrestricted registered professional nursing license BEFORE July 1, 2020 in New York State, another state or United States territory:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
For individuals issued a registered professional nursing license on or AFTER July 1, 2020, based on expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the Laws of 2018:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of
Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or university, registered with the NYSED; or
b. Obtains a Bachelor’s of Science Degree in Nursing or higher, as described in “a” above, within ten (10) years of the date of initial licensure; and
2. Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

YEARS OF EXPERIENCE:Over 1- 2 years
