Job Description

The Penn Medicine Hospice Team Manager is responsible for the financial and administrative operations of the hospice nursing team for Penn Medicine Hospice. Additionally, s/he will be responsible for the continuous development of Penn Medicine Hospice and Penn Home Palliative Care. The Team Manager will assist with the development of other Penn Medicine at Home shared services such as Home Care Central Intake and Admissions process. Along with operational responsibilities, the incumbent will develop and manage the activities of the hospice and palliative nursing teams and support the development, implementation, evaluation, and coordination of hospice nursing practices and policies across all Home Care business lines.

Bachelor's Degree Nursing and 3+ years management experience, preferably in a Home Care/Hospice/Health Care facility. Graduate of an NLN approved
school of professional nursing required.

Master's Degree preferred

Demonstrated comprehensive knowledge of accrediting and regulatory agencies including CMS, OASIS and Joint Commission.
Computer applications, equipment software to support patient care delivery

Licensed as a professional nurse in the state of Pennsylvania and/or New Jersey depending on geographic location and scope of responsibilities.
BLS, Valid Drivers License and Auto Insurance required.

Ability to demonstrate the knowledge and skills necessary to provide care based on physical, psychosocial,
cultural, educational, safety and related criteria appropriate to the age of the patients served in his or her
assigned area.
Demonstrates complete understanding of DOH and JCAHO regulations.
Evidence of excellent interpersonal skills and communication skills.
Strong written and oral communication skills.
Must be success-driven and results-oriented.
Must be an analytical thinker with a keen understanding of the effective use of data via project/process
Ability to navigate electronic medical records and various health system databases.
Strong problem solving, brainstorming, and planning skills.
